USA HOCKEY REGISTRATION REQUIREMENT

USA Hockey registration fees are NOT included in the Full Travel Fees - All Players, Coaches and Managers are required to complete and pay for their own 2010-2011 USA Hockey registration. All Players MUST provide their USA Hockey confirmation number at the time of Royals registration. For players born in 2005 or later, registration is free, but still required in order to participate.

Download and print forms to fill out and instructions to follow. Required forms are:

1) Royals 2011-12 Application

2) DVHL Player Agreement

3) USAH Code of Conduct

4) USAH Consent to Treat

5) USAH Waiver of Liability

Also print out and follow USA Hockey Registration Instructions. Note:

All new players must present an original (raised seal) Birth Certificate for proof of age. Photocopies of Birth Certificates are not acceptable. Bring completed forms and NON-REFUNDABLE, NON-TRANSFERABLE deposit of $300 to the Wintersport office. Full Travel teams may fill and registration may be closed for a level, therefore, Full Travel registrations must be approved by coach and club before being accepted and processed.
